Well-Nourished vs. Well-Fed: How We Assess a Child’s Nutritional Status

Pediatric gastroenterologists hear this almost daily. However, when talking about a child's nutritional status, the assessment isn't based solely on how a child looks or how much they weigh. It involves a systematic evaluation of growth, body composition, food intake, clinical condition, and, when indicated, laboratory parameters.
Modern pediatrics and recommendations from the ESPGHAN (European Society for Paediatric Gastroenterology, Hepatology and Nutrition) and ESPEN (European Society for Clinical Nutrition and Metabolism) emphasize one key point: nutritional status is much more than a number on the scale. It is a vital indicator of a child's health, growth, and development.
How Do We Objectively Assess Nutritional Status?
During an exam, we evaluate body weight, height, and BMI, while head circumference is monitored in infants and young children. These measurements are compared against standardized growth charts and expressed using percentiles and Z-scores. This allows us to assess not only if a child falls within the expected range for their age and sex, but also how far they deviate from the population average.
Why Is Growth Velocity Key?
Monitoring how fast a child grows is just as important as measuring their current weight.
“For example, a child who has been in the 10th percentile for years may be completely healthy as long as they follow their growth curve steadily. That is simply due to their genetics!” says pediatric specialist Dr. Nevena Popovac. “On the other hand, a child who previously grew along the 75th percentile and then suddenly drops off their trajectory requires careful evaluation and monitoring, even if their current weight still falls within the reference range.”
What Can Compromise a Child's Nutritional Status?
Nutritional status depends on far more factors than just the quantity of food on a plate.
When Is Picky Eating Just a Phase, and When Does It Become a Problem?
In younger children, picky eating is often a completely normal, temporary developmental stage. Between the ages of two and six, many children become wary of new flavors, stick to a small group of favorite foods, or go through periods of reduced appetite. If the child continues to grow and develop normally, this behavior is usually no cause for concern.
However, in a small subset of children, restricted food intake becomes severe enough to cause nutritional deficiencies, weight loss, slowed growth, dependence on supplements, or significant disruption of daily life. In these situations, Avoidant/Restrictive Food Intake Disorder (ARFID) should be considered.
Being Well-Fed Is Not the Same as Being Well-Nourished
A child can have an average or even higher body weight while simultaneously suffering from specific micronutrient deficiencies. Sufficient caloric intake does not guarantee adequate intake of all essential nutrients. “In other words, a child can feel full and weigh enough without being nutritionally well-cared for,” warns Dr. Popovac.
Restrictive Diets
Children following restrictive diets require special attention. An increasing number of families today choose a vegetarian or vegan lifestyle. A well-planned vegetarian diet can meet a child's nutritional needs, whereas a vegan diet demands careful planning and a reliable source of vitamin B12, typically through supplementation. Special attention must be paid to the intake and status of iron, calcium, vitamin D, iodine, zinc, and omega-3 fatty acids. Professional guidance, growth tracking, and risk assessments for nutritional deficiencies are strongly recommended for children on restrictive diets.
Modern Diagnostics and Treatment
Assessing nutritional status doesn't end with physical measurements. A pediatric gastroenterologist carefully analyzes dietary patterns, meal frequency and composition, eating habits, potential digestive issues, physical activity levels, and the child's psychosocial environment.
Based on this information, we decide whether targeted blood work or further diagnostic tests are needed. Laboratory work should never be routine or identical for every child; it must be tailored to the clinical presentation and assessed risk.
Treatment is Always Individualized
Sometimes, educating parents and gradually introducing new foods through play is enough. For other children, medical supplementation of vitamins and minerals, oral nutritional supplements, or consultations with a clinical dietitian may be necessary. For children with severe feeding disorders, a multidisciplinary approach involving a pediatric gastroenterologist, pediatric psychologist, speech therapist, occupational therapist, and dietitian is essential.
The goal isn't simply getting a child to eat more food, but ensuring an adequate intake of energy, protein, vitamins, minerals, and other essential nutrients required for healthy growth and development.
When Food Becomes Medicine: Celiac Disease and Crohn's Disease
In pediatric gastroenterology, nutrition isn't just an aid in maintaining health. For certain conditions, nutritional therapy forms the foundation of treatment and can act as a primary medical intervention.
Celiac Disease
In patients with celiac disease, even tiny amounts of gluten trigger an immune response that damages the lining of the small intestine.
- Onset and Triggers: Gluten triggers the disease in genetically predisposed individuals, but its etiology is complex. Symptoms can appear at any point, from early childhood through adolescence and into adulthood.
- Treatment: A strict, lifelong gluten-free diet is the only effective treatment. It allows the intestinal lining to heal and normalizes nutritional status and growth.
Crohn's Disease
Another example is Crohn's disease, a serious chronic inflammatory bowel disease.
- Onset and Triggers: It develops from a combination of genetic predisposition, altered immune response, and environmental factors. It is most commonly diagnosed in adolescents and older children.
- Treatment (EEN): For children with newly diagnosed Crohn's disease, Exclusive Enteral Nutrition (EEN) is a primary therapeutic option for inducing remission, as well as for managing relapses, particularly in luminal disease. For a set period, the child receives all their nutritional needs through a standardized liquid formula, avoiding solid food entirely. EEN can reduce disease activity, improve nutritional status, and induce remission without exposing the child to corticosteroids.
Strict Elimination Diets vs. Food Intolerances
Parents need to understand the difference between chronic conditions requiring a strict elimination diet and intolerances:
- When a child has a chronic autoimmune condition like celiac disease or a proven allergy with an anaphylaxis risk, the elimination diet must be absolute. With celiac disease, there is no safe threshold for gluten, making strict avoidance—including precautions against cross-contamination—essential for life.
- With intolerances, such as lactose intolerance, the underlying mechanism is different and does not involve immune-mediated gut damage. The level of restriction depends on individual tolerance and rarely requires complete, lifelong elimination of the food item.
